Neurology comprises the brain, spinal cord and the peripheral nerves. Diseases of brain and spinal cord can be severe. Most common scenario is an unconscious patient after major brain diseases such as brain hemorrhage of after traumatic brain injury. Such patients need strict monitoring of consciousness level and vitals like heart rate, respiratory rate, blood pressure etc.
Critically ill patients poses various kind of challenges, when it comes to their care and treatment. A majority of these patients are in deep sleep like state. Thus, their position needs to be changed every 3 to 4 hours to prevent pressure sores. Limbs and chest physiotherapy is vital part of neuro-critical care. Chest infections are common in such patients. Thus, airway and breathing need to be monitored frequently. Some patients need tracheostomy that is creation of small hole for tube insertion inside the wind pipe. Ventilator or breathing machine is needed in some patients. All vital measures to avoid and treat infection are taken in neurocritical ICU. This is because infection badly impacts recovery of critically ill patients and increases ICU stay.
Patients might be shifted to the hospital ward or a rehabilitation center once improvements are made during a stay in ICU. Patients who have suffered critical illness need significant time and a good deal of rehabilitation and physiotherapy to continue to recover to normal life.
In normal cases, endoscopic spine surgery is done. In our hospital, a special instrument known as endoscope is used to reach deeper spine structures such as intervertebral disc. It is used mainly in slip disc surgery. Through a small incision this tube like instrument is inserted in back. A small camera attached at the tip of endoscope assists in viewing all important structures such as nerves, veins etc. Thanks to the help of special instruments, herniated part of disc is removed to relieve pressure over the nerves.
Since there is minimal invasion and less tissue trauma when surgery, patient can be mobilised on the day of surgery.
Also, high speed drill is used. This is special type of drill machine helpful in surgeries involving cutting the bony skulls and other bones of the body. It is helpful in spine surgery where the vertebral body bone has to be removed.
